A multiparous client births a 9-lb newborn at home. Ten minutes after an intact placenta delivers, she has a sudden gush of blood soaking two pads. The midwife palpates the fundus and finds it soft and above the umbilicus. Vital signs are stable so far. What is the most appropriate FIRST action?

  • A. Wait 10 minutes to see whether the bleeding slows on its own
  • B. Start external aortic compression
  • C. Immediately give methylergonovine IM
  • D. Massage the fundus while beginning to empty the bladder
Why

A soft, high, deviated fundus with brisk bleeding points to atony, often worsened by a full bladder. The first step is fundal massage (and emptying the bladder), which is immediate and frequently effective. Uterotonics follow if massage doesn't control it; aortic compression is a later, escalation measure. Waiting risks rapid decompensation.
